Details:
- Permanent, full-time position
- Provide independent care in nursing facilities, home visits, hospital visits, and acute care facilities
- Generate initial assessment and Care Plan
- Conduct Home Safety Assessment and Comprehensive Chronic Condition Assessment
- Travel throughout Bay Area required
- EMR: Epic

Qualifications
- Board Certified
- Valid California Nurse Practitioner or Physician Assistant license
- Minimum of one year of clinical experience in Geriatrics, Palliative Care, or Internal Medicine
- Home visit experience required
