The Opportunity (Login for Information) seeks part-time or full-time Nurse Practitioners for our San Diego, California market. Join us to work with a focused patient panel of complex care cases in a value-based model where care navigators help maximize provider effectiveness.

What You'll Do

• Deliver onsite primary care to seniors in living communities
• Create person-centered care plans
• Collaborate with Medical Director on policies and guidelines
• Conduct monthly Chronic Care Management visits
• Lead clinical team huddles
• Partner with senior living staff to improve outcomes

What We're Looking For

• Nurse Practitioner (geriatrics experience preferred)
• Comfort with fast-paced environments
• Willingness to provide care across Southern San Diego
• Passion for complex care and holistic approaches
• Collaborative mindset with multidisciplinary teams
• Strong communication skills

Compensation Base salary: $135,000-$165,000 plus equity, benefits and performance bonus opportunities.
