Position Overview

As part of (Login for Information), you’ll have the opportunity to work closely with a team of dedicated, friendly providers and staff. Good sense of humor Our small, close-knit clinic fosters deep, long-lasting relationships with patients and offers a unique, community-oriented care setting. We are searching for a compassionate and skilled Primary Care Provider to join our team and continue our tradition of exceptional care. This position will be receiving a number of patients from one of our providers who is transitioning to a Psychiatric Nurse Practitioner role in the clinic. Your day will include seeing these existing patients, establishing new patients, and treating PRN walk-ins, up to 20 or so appointments per day.
Provider Responsibilities
Responsibilities include, but are not limited to:
- Conducting thorough, comprehensive patient assessments, including history-taking, physical exams, diagnosis, and management of common primary care conditions
- Creating individualized, evidence-based treatment plans and providing follow-up care as necessary to monitor progress and adjust treatments
- Collaborating with other providers and support staff to ensure a seamless, coordinated approach to each patient’s care
- Educating patients and their families on health maintenance, preventive care, and managing chronic illnesses, with a focus on patient empowerment and shared decision-making
- Maintaining meticulous, accurate, and complete patient records to ensure continuity and high standards of care
- Managing prescriptions, monitoring medication effectiveness, and advising on best practices for medication adherence and safety
- Engaging in professional development to keep current with best practices, clinical guidelines, and advances in primary care
- Demonstrating empathy, professionalism, and dedication in all patient interactions, fostering trust and respect within our community
- Actively participating in clinic initiatives, quality improvement efforts, and team meetings to enhance clinic operations and patient satisfaction
- Maintaining and contributing to (Login for Information)’s positive reputation by delivering outstanding patient-centered care and upholding our clinic’s values of compassion, integrity, and community focus
Patient Load
- Caseload: 20 to 22 patients per day
- Schedule: Monday through Friday, days, currently ~4 x 10 hours/week
- On-Call: Minimal and scheduled in a manageable rotation
Qualifications
- Licensure: Current, unrestricted Nurse Practitioner license in the state of New Mexico
- DEA License: Active DEA license required
- Certification: Board-certified as a Family Nurse Practitioner (FNP)
- Experience: At least one year of experience in a primary care setting is preferred, but new graduates are welcome to apply
