We seek a skilled and compassionate Family Nurse Practitioner (FNP) to join a new advanced primary care health center in Liverpool, NY. This is a solo provider site, supported by one Medical Assistant (MA), and will serve a patient population of 300-500 individuals. The role involves managing a range of health conditions, with a focus on chronic disease management and some occupational medicine (including DOT physicals). If you are passionate about prioritizing patient care over quotas and focusing on well-being rather than billing, this is the perfect opportunity for you!
Key Responsibilities:
- Provide primary care and chronic disease management
- Conduct DOT physicals and other occupational health services
- Work collaboratively with the Medical Assistant to ensure high-quality patient care

Qualifications:
- Experience in chronic disease management and occupational health preferred
