Nurse Practitioner Philadelphia, PA
We are looking for a compassionate Nurse Practitioner to join our Medical Clinic Full time in Philadelphia, PA.
The Nurse Practitioner must be caring and empathetic, have great listening skills, detail-oriented, with some basic computer skills and knowledge of Microsoft office suite. The office hours are 9am-5pm Monday-Friday (9am-7pm Thursday) and we are looking for someone to work between 3-5 days a week.
We are looking for a dedicated Nurse Practitioner to serve the needs of our clients in alignment with our mission at our Wellness and Family Planning Clinic. The Nurse Practitioner will independently assess acute and non-acute clinical problems, perform and document physical assessments and patient histories, analyze trends in patient conditions and develops documents and implement a care management plan in response to the data obtained. This position also includes facilitating and collaborating with other clinical staff for the development of the plan of care as well as providing appropriate patient/family/significant-other counseling and education. The NP will work on an autonomous basis while having an off-site medical director for supervision.
About us:
Our Wellness center is a Family Planning clinic that serves the immediate community on a walk-in and as-scheduled basis to provide contraceptive services, pregnancy counseling, reproductive counseling, STI/HIV services and treatment and preventative care for various chronic health issues that affect our served population. Our clinic has been proudly serving the community for over 3 years and we continue to grow.
Duties/Responsibilities:
- Plans patient care based on in-depth knowledge of the specific patient population by anticipating and identifying physiological, psychological and socioeconomic problems commonly encountered in the served population including the consideration of the patient’s cultural background, level of understanding, personality, and support systems
- Serve as a patient advocate
- Writes admission, transfer and discharge orders
- Orders and interprets appropriate laboratory and diagnostic studies
- Prescribe appropriate medication and treatments
- Refer patients for consultation when indicated i.e. prenatal care, chronic disease management, infectious disease control, psychiatric or mental health care, social services, dietary and nutrition counseling, etc.
- Documentation, in-depth progress notes and summaries on electronic health record system
- Perform contraceptive education, pregnancy planning counseling and STD testing and treatment and HIV care/treatment
- Communicate patient management strategies to Practice Administrator and Medical Director
- Collaborate with members of the staff when appropriate to ensure interdepartmental collaboration while ensuring HIPAA compliance to develop patient management strategies that are successful in meeting patient care needs (ie. case management, prevention services, hunger-relief services etc.…)
- Recognize situations that require the immediate attention of a physician and initiate life-saving procedures when necessary
- Participate in outside activities that enhance personal and professional development and foster positive community health outcomes and community outreach
KNOWLEDGE, SKILLS AND ABILITIES:
- Current RN licensure and PA Nurse Practitioner Certification (CRNP) required
- Minimum 3 years clinical experience as an NP providing direct patient care, HIV treatment experience preferred
- Excellent communication and interpersonal skills
- Basic computer skills and some knowledge of Microsoft Office Suite
Schedule:
- The office hours are 9am-5pm Monday-Friday (9am-7pm Thursday) and we are looking for someone to work between 3-5 days a week
