Two locations - one alongside 5 providers and other is solo - whichever suits you best
Outpatient only, ramping up to seeing 18 patients per day
Ongoing salary plus quality metrics annual and $3,000 for CME expenses
Sign-on and Relocation Reimbursement, FQHC for Loan repayment program
