Physician Assistant Nurse Practitioner Vancouver, WA
We are looking for a Physician Assistant or Nurse Practitioner to join our growing medical team part time in Vancouver, WA. We are looking for a provider who can bring compassion and passion, energy and focus to their work every day.
This role requires providing geriatric and adult primary care to patients in the Vancouver and Clark County area. You’ll visit patients in their homes, both in senior communities, nursing facilities, and long-term care settings and work with the facility staff to deliver high quality care on-site. This role is for an experienced PA/NP who has autonomy in clinical decision-making, without the need for continuous MD supervision and oversight.
Duties:
- Provide preventative and primary care for patients at home and in skilled nursing facilities
- Management of acute/chronic conditions such as Diabetes, CHF, and Hypertension
- Issuing appropriate orders, prescription refills
- Utilizing a comprehensive EHR to manage patient care
- Communicating effectively and promptly with team members, patients, caregivers and families via phone, email, and fax
- Maintaining accurate and complete documentation of all clinical information and updating patient records in a timely manner
- Collaboration with the nursing staff for comprehensive care
Requirements:
- Graduation from an accredited Physician Assistant or Nurse Practitioner program (PA/NP)
- Current PA/NP license to practice in WA
- Experience with/clinical background in geriatric and adult primary care required
Schedule:
- Approx. 25 hrs per week
- Rounding at senior care facilities (several days per week)
- Charting done remotely (home)
Benefits:
- Bonus Compensation- 5% salary quarterly for same day chart closure rate of 90%
