Job Description
Are you a new grad or experienced post-acute care nurse practitioner (NP) or physician assistant (PA) who aims to make a difference? (Login for Information) needs someone like you! We are seeking an NP or PA to work in a skilled nursing facility rounding 2-3 days/week. This opportunity includes the flexibility to set your own schedule. We have no start/stop times so you can round in the morning or in the afternoon and still have great work-life balance.

Key Responsibilities:
- Perform primary care in a skilled nursing facility setting for short term and long-term patients
- Manage chronic disease, diagnose illnesses and medical conditions, and implement plan of care
- Order and interpret lab results
- Prescribe and monitor medications
- Consult with physicians and coordinate patient care with other healthcare providers
- Educate patients and their caregivers on coping skills to handle chronic and age-related conditions
- Build relationships with your patients, patients' family members, and facility staff to facilitate effective treatment delivery
- Promote healthy lifestyle changes
- Critical thinking skills
- Excellent communication skills
- Ability to collaborate with the healthcare team
- Familiar with EMR and billing/coding
- Strong clinical skills, work ethic, and ability to work independently
- Adaptability and flexibility to address the fluctuations that occur during the aging process and with chronic illness
- Empathy, compassion and ability to connect and engage with your patients.
