- Rounding between 2 facilities available 4-5 days a week on a full-time basis to start in area
- Patient volume 15-20 patients/day
- Provide primary care services in post-acute care facilities, including skilled nursing and assisted living
- Drive high-quality, safe, and cost-effective patient care
- Collaborate with interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Provide oversight, guidance, and mentorship to clinicians within facilities
- Background in providing primary care in internal medicine, family medicine, emergency medicine, urgent care, physiatry, PM&R, or geriatrics
- Ability to work flexible schedules
