Excellent opportunity for established primary care Advanced Practice Registered Nurse with strong outpatient pediatric experience with a passion for serving looking to join a small, independent clinic.
(Login for Information) (SFWC) approaches health from a strengths-based, trauma-informed perspective that is driven by those we serve. In addition to evidence-based pediatric care, we offer behavioral health and healthy living support for the entire family.
Position to include Vaccine for Children coordination, shared call and community outreach. We are seeking the right person to join our integrated care team in either parttime or full-time capacity. Located in Louisville’s historic first African American neighborhood, Smoketown is designated as a medically underserved area, with the possibility of loan forgiveness. Panel size is limited to allow for quality patient care. Hospitalist service provides inpatient care. Access to University of Louisville specialist is available. There is an opportunity to be involved with research, if interested.
Education and Requirements: APRN degree with national certification and current, valid, and unrestricted licensure in the Commonwealth of Kentucky. Must remain eligible to be insured for medical liability at all times. Two years of pediatric clinical experience. Professional references, background check, urine drug screen required. As a condition for employment, you must be vaccinated or be willing to be vaccinated for influenza and COVID.

Our Center has a teaching kitchen, behavioral health services, after school programming and prescriptive food pantry and community garden to support families in need.
